Should I let a marriage go after 15 years?

Unfortunately I can not answer that question for you but I can give you some suggestions of some things to think about. This question is one that only you will be able to answer for yourself.
It sounds like you have been through a lot and have battled hard to get to where you are at today. When you experience a life or death experience, it can really change your world view and change your perspective. I think it is wonderful that you are at a place in life that you want to live a more fulfilled life. That is what life is all about, really living and making sure you do the things you aspire to do while you are on this earth. If we get to a point in life that we decide to prioritize ourselves, in a good way for our own well being, you start to realize that healthy boundaries and positivity is a must. I would be curious in what a "more fulfilled life" looks like to you. What does that consist of? What changes would happen within yourself? What things would you be doing? Think about what that means to you.
The other question would be, would your partner be open to joining along side of you with some of what that is? Does your partner understand what that means for you or do they have something completely different in their mind of what that is for you. Is this a relationship that you can compromise on some things if they can compromise as well? You mentioned that they request that you stay the same. I would be curious of what about "a more fulfilled life" are they worried about for you, what is driving them to not want you to make a change that would be good for your wellbeing? These are just a few things to think about that may help you with some exploration of the question that you are trying answer for yourself.
It is always important to try to give the relationship and the two of you the opportunity to communicate about these things together. If you talk about it, you may find that there are misunderstandings or strong feelings that might be a barrier to moving forward. If you talk with your partner too, that may also help you with thinking about what your next steps may be. You will also know that you have tried to communicate and hope to understand why it is they have made that request, knowing that for you, this is something you want to do to be a better you and live a more fulfilling life.
