About me

I am a clinical social worker with over 20 years experience working with an array of mental health concerns, including depression and anxiety. I have worked in emergency rooms, as a shift manager at a crisis hotline, psychiatric hospital units, medical clinics, and ran my own private practice. I have been licensed since 2001 in the state of Washington and my credentialing license number with the State Board of Health is LW00005238. I believe everyone should be treated with respect and dignity, and counseling is a partnership to achieving your goals. I view my role as helping one understand themselves, and help identify choices and options to reach goals and move forward. The modality that I use the most in counseling is Cognitive-Behavioral Therapy (CBT), but will also use skills from other modalities, including Psychodynamic Theory, DBT, and Acceptance Therapy.

Telehealth Counseling with Lisa Berendts, LICSW, MSW Consent

1. Telehealth Counseling:  Telehealth counseling allows therapists, clinicians and specialists to provide a diagnosis, consultation and treatment using videoconference technology via telephone or computer.
2. Purpose : The purpose of Telehealth Counseling Consent Form is to get permissions from patients in order to participate into telehealth counseling services.
3. Confidentiality Audio and video recordings of patients during appointments/online visits will be kept private. Medical and personal information of patients are protected by the state and governmental laws.
4. Risks and Benefits:   Telehealth counseling aims to provide a complete treatment to patients. It is considered and supported by researches that online counseling is an effective way in treatment of various disorders, personal issues and problems. On the other hand, it may not be beneficial and there is no guarantee that the counseling will be effective for patients.
5. Payment: Counselors receive no information about what plans have been purchased through BetterHelp. All inquires about payments/financial issues will be referred to BetterHelp.
6. Rights:  Patients can withdraw and withhold this consent at any time and they can end the treatment any time they would like to. Any action of patients will not affect the future treatment of or accessibility to counseling services.
7. No shows: After two No Shows for appointments, or appointments cancelled without 24 hours notice, Lisa Berendts has the right to discuss termination of services or suggest transferring to another therapist.
8. After 3 weeks of no contact, it is assumed that client has terminated counseling with Lisa Berendts, MSW, LICSW. It will be left to the client to resume counseling if interested.
9. Consent by agreeing to this form, I agree that I know and understand the information above. My questions were answered completely during the discussion with the therapist, clinician or specialist. I hereby give my consent to participate into telehealth counseling services provided by Lisa Berendts, MSW, LICSW
