About me

Having experienced the benefit of being born and raised in Brooklyn, NY I was gifted with exposure to a varied cultural base. My first work engagement in the mental health field occurred in 1996. The facility was a medicaid based dual diagnosis treatment center with the primary diagnoses psychiatric with a secondary substance use disorder. My interactions with the patients flowed naturally and, in my discussions with staff, the owner of the facility suggested that I should get my license in mental health counseling. I began graduate study in 1998 while working full time in varied facilities in order to gain maximum exposure to diverse mental health/substance abuse modalities. My work experience included the Palm Beach County psychiatric crisis stabilization unit, The Drug Abuse Foundation in Delray Beach and the Juvenile Intervention Facility in Broward County. Upon graduation in 2000 and license activation I spent time as a therapist at the Renaissance Institute in Boca Raton. Shortly after the events of 9/11/2001 I began the process of creating a private practice in psychotherapy. During the growth of my practice beyond seeing patients for individual therapy, I provided services for EAP programs, sober house residences and treatment facilities. In December 2010 I closed my practice and relocated with my dogs to western North Carolina. I spent ten years at Pavillon in Mill Spring, NC as a clinical supervisor until November 2020. I continue to reside in North Carolina and hold active licenses in Florida (LMHC) and North Carolina (LCAS). I am experienced with a varied patient demographic and am easy to connect with. While currently semi-retired I continue seeking to help provide guidance and direction for others.

Thomas Dukes is an excellent therapist.
He is well trained and has a wide range of life experience and knowledge which makes him a very valuable companion through life's ups and downs.
His open, non-judgmental and compassionate approach quickly builds trust and makes it easy to open up to him.

Thomas has been my therapist through some really difficult life events. And even in the hardest moments, with his support by my side, I never lost hope for better days to come.
Life is still not easy, but through his guidance I have learned to cope better with difficult challenges. With his support, I now enjoy the good things in life despite the difficulties along the way. And I recognize my resilience and believe that I am capable of growing through challenges.

In general, Thomas has empowered me to change my life for the better and I have achieved goals that seemed far out of reach and impossible.

Thomas Dukes is wonderful. Top notch! He listens and understands what I need help with and guides me in the right direction. He is always helpful and is never judgement. I would recommend him to absolutely anyone!

I like Tom simply because I think he gets me. I can dump out my thoughts and he gets it.
I think his background is a bonus for an older single guy struggle to find my way forward is a good thing.

Tom effortlesslessly prances across the fine line between providing non-judgemental space for one to share openly and taking no shit. He's neutral to information in a way that only someone with experience can be, so I find it incredibly easy to disclose the tough stuff. However, if you're just looking for a passive listener on which to unload, Tom is not your guy. His focus is on actionable steps. If you're open to hearing what you need to hear and answering potentially uncomfortable questions you need to consider order to make changes you've been saying you want to make, you'll find a good match here.
