Can you prescribe medication if needed?

I am on Effexor XR. If I need to change meds I wanted to know if you can prescribe some thing different. Ive been on Effexor for 30 yrs. But they dont seem to be working anymore

In my role as a counselor, I am not able to prescribe medications.
