Typically, within the first session the client and counselor will discuss whether the client is actively being treated by another health care professional. During this time, a client may include as part of his treatment goal engagement with other health care providers in which the client is currently being seen or treated. It is not uncommon for a client to have multiple professionals of different disciplines and specialty, as a part of their overall treatment team. Most counselors’ welcome additional professionals who are a part of the healing process and well being of the client. In addition, the client at anytime during the counseling relationship can request consultation with other providers. The client can sign a written request to authorize release of information for both his or her therapist and physician. The written authorization should specify what information is to be released, to whom it is to be released and what time-period should be implemented for release of information. Having a support system of multidisciplinary professionals can certainly help to inform decision making and benefit the client in establishing treatment or discharge planning within the community. Pursuit to the Code of Virginia, Title 32.1 Health Records Privacy, chapter 5. Regulation of Medical Care Facilities and Services “Health care entities shall disclose health records to the individual who is the subject of the health record, except as provided in subsections E and F and subsection B of 8.01-413”. In other words, the health care provider upon request of the client who is the person of record shall disclose to other health care entities in any available format requested by the client. In short to answer the question, the client can make a written request to a therapist or health care provider for release of information in any available format of their choosing, whether it be verbal, written, or by electronic exchange. In addition to helping the client strengthen his or her support within the community setting. The therapist is better able to serve the best interest of the client when engaged with multidisciplinary professionals. In terms of advocating for the client and linking them to additional services.
